The story picks up a year after Agnese 's death . Andrea returns to Milan to restart his life, but finds a hospital he barely recognizes. Among the cases is that of Dante Pellegrini , a doctor suffering from pulmonary fibrosis, and his son Matteo , a young resident admitted to the Ambrosiano.

A sudden death from excessive morphine leads Andrea to suspect that someone is causing the patients' deaths. The cast also includes Cristina Marino (Lidia Morganti), Argentero's real-life wife, and Andrea Pennacchi.
